When picking your healthcare, it is important to have options. That is exactly why we created "Choice Plus" from United National Life. We can tailor a policy that fits your needs, and exclude the benefits that you don't need.

Did you know the average hospital stay is less than seven days (regardless of your age)? So do you really need a supplemental policy that covers you for 365 days or more? With Choice Plus you decide if you want to be covered in the hospital for 10 or 20 days, as well as the amount you want each day you are confined in a hospital...up to $500 per day. The base policy will also pay you $30 every time you go to the doctor up to 20 times per year.

This policy lets you decide what is important. Buy only the benefits that you feel are useful to you. Why waste your hard earned money on benefits that you most likely will never use? Choice Plus gives you the freedom to choose.

Base Policy

10 or 20 day hospital confinement up to $500 per day. (Choose from $100 to $500 per day)
20 doctor office visits per year reimbursed at $30 per visit. (One visit per week)

Optional Benefits

Increase your doctor office benefit to $40, $50, $60 per visit
Dental/Vision... After $100 deductible, we will pay 80% of dental or vision expenses up to $750 annually
Outpatient Surgery... Choose from three levels of reimbursement for Outpatient Surgery...$500 to $1500
Emergency Room and Ambulance...$200 twice a year for both Emergency Room and Ambulance
Cancer Coverage...$2500 to $5000 of lump sum coverage on first diagnosis of Cancer
Skilled Nursing Care... $75 or $150 per day for skilled nursing care starting on day 21

All benefits are paid directly to you.

Guaranteed renewable for life. As long as your premiums are paid, you can not be canceled.

Stable premiums. Your premiums can not change due to declining health like other insurance policies. Your premiums can only change if we change the premium for everyone in your state on the same policy.

Payment grace period. You have 31 days after your premium is due to keep your policy in force.

10 day free look. If you are not completely satisfied with your policy you can return it within 10 days for a full refund of your premium.
